Consent

I agree and consent to assessment and treatment. I understand that my personal health information will be collected. I understand that all information I provide will be kept confidential unless required by law. I understand, and consent that my medical information may be shared by the various care providers involved in my care and treatment Treatments may be covered by extended health care plans. I understand that it is my responsibility to confirm the exact details of my coverage. 

Appointments

A credit card is now required on file to hold your scheduled appointment. When booking online, you will be asked to input a credit card before you can schedule the appointment. The card WILL NOT be charged when scheduling online. Those that schedule in the office will be required to have a credit card on file via our secure Square system. If the client fails to comply with Cancellation/No Show policies below, then the credit card will be charged. There are no refunds given on services rendered.

Cancellation Policy

We respectfully ask that you provide us with 24-hour notice of schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing 24-hour notice, we are often unable to fill that appointment time. This is not only an inconvenience to your therapist but to other potential clients as they are unable to schedule for services they need. For this reason, you will be charged 50% of the service fee. We require a credit card number to be given to book future appointments so that appropriate fees may be charged if a late cancellation does occur. We understand that emergencies can arise and illnesses do occur at inopportune times. If you have a fever, a known infection, or have experienced vomiting or diarrhea within 24 hours before your appointment time, we request that you cancel your session. Inclement weather may also result in the need for late cancellations. We will do our best to give advance notice if we are closing or need to cancel due to bad weather, and we ask that you do the same. If we are unable to answer the phone, please leave a voicemail with your cancellation request. Our voicemail is time-stamped, so we will be aware of the time you called. Contact us at 919-548-8459. No-shows will be charged 100% of the service fee. 

No Shows/Late Cancellations

Anyone who fails to show up for a scheduled massage appointment and does not provide a cancellation notice will be considered a no-show and will be charged the full amount for the missed appointment via the credit card on file. Payment must be received before scheduling your next appointment.

Late Arrivals

Out of respect and consideration to your therapist and other customers, please plan accordingly and be on time. If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment given, you will be responsible for the full session.

Gift Certificates/ Packages

Anyone who is using a gift certificate or pre-paid package for their session and is either a “no-show” or “late cancels” for their appointment, will be charged that gift certificate or pre-paid session. All gift certificate sales are final, we do not offer refunds on unused gift certificates. You can check gift card balances or purchase a new gift card. All gift card and package sales are final.  There are no refunds given on gift card purchases or package sales.

Price Increases

At Functional Bodywork Therapy, LLC when/if a price increase for any service occurs, the increase will be effective on the specified date that Functional Bodywork Therapy, LLC has determined.  Clients will be given at least 1 month’s notice of the price increase.  Clients will be made aware of the price increase via email correspondence, and postings within the office.  When the price increase goes into effect on the specified date, prices of all currently booked sessions and future sessions will increase on the effective price increase date.  

COVID- 19 Policy

Any client who has tested positive or been exposed to anyone who has tested positive will have their appointment rescheduled for 10 days after testing positive or the date of exposure.  We have many clients with comorbidities that are at a higher risk for contracting COVID-19.  This policy is in place to protect our team and our clients.

Inappropriate Behavior Policy

Massage therapy is for relaxation & therapeutic purposes only. There is absolutely no sexual component to massage whatsoever. Any insinuation, joke, gesture, conversation, or request otherwise will result in immediate termination of your session and a refusal of any and all services in the future. You will be charged the full-service fee + 25% gratuity regardless of the length of your session. Depending on the behavior exhibited, we may also file a report with the local authorities if necessary. We ask that you treat your therapist with respect and dignity as we do our clients.
